7 Day Diary of a Chemical Peel

7 days ago I decided to do a chemical peel. Why, because I know that chemical peels are an excellent way to refresh and rejuvenate the appearance of the skin. And who doesn't want their skin to look refreshed and rejuvenated, especially at this time of the year. So with some trepidation I asked Allyson, the PA at Timeless to treat my skin with a chemical peel. Why the trepidation you ask? Because I did want my skin to look crazy!

Day 1: Allyson applied the chemical peel at the end of my work day. It's a quick and fairly simple process that involved Allyson washing my face and applying several layers of a chemical solution. During the entire process she check pointed with me to insure that I was comfortable. Within a timeframe of about 10 minutes I was done with instructions that I should wait about 5 hours and then I could wash the chemical solution off my skin. In that first 5 hours I did experience some itching, however it was nothing that a glass of wine didn't help.

Day 2: My face appeared a bit pink but other than that I don't think anyone would know that I had done a chemical peel one day earlier. I passed on wearing make up and instead just applied SkinMedica's Ceramide cream.

Day 3: A repeat of day 2.

Day 4: Let the peeling begin. I first started to peel around my mouth and on my chin. All fears were diverted. I did not kook like some reptile shedding their skin, instead my skin was just very lightly flaking off. I was in the office but choose not to wear makeup. Again, I just kept my ceramide cream close by to conceal my flaking skin.

Day 5: More flaking but still not in the fashion of a reptile.

Day 6: With most of my peeling complete I decided that my Clarisonic brush was the perfect answer to loosen and remove any remaining peeling skin.

Day 7: Results as advertised! My skin is glowing. I have that fresh, dewy look to my skin. Would I do it again? You bet. Will I share my secret with my friends? Of course! Am I going to make this part of my maintenance routine for my skin? Why wouldn't I? A chemical peel is quick, painless, affordable and produces results!

A "Timeless" Weekend

A few weeks ago I had an incredible opportunity to attend a conference in Carlsbad, California for 2 days of meetings with SkinMedica. For those of you who are not familiar with SkinMedica, you may want listen up! SkinMedica is one of the favorite skin care product lines of Dr. Clinton and the staff of Timeless Skin Solutions. This is the company that makes Essential Serum…you know…The Essential Serum. As seen on Oprah and The Doctors! I know a few lights just went on and ears perked up!

Why is SkinMedica one of our favorites?

The quick answer is because these products make a difference in the appearance of the skin! It is our goal at Timeless Skin Solutions to enhance and rejuvenate the look of our patients and these products make that goal attainable.

I do NOT travel much, which is why I packed a small suitcase for my conference with what I thought would be the bare minimum for three nights. I had a few business casual dresses, the token workout clothes and bathing suit, which were never used, my hair straightener, a bag full (okay two bags full) of toiletries, makeup and my Timeless skin care products like my favorite SkinMedica Aha Bha cream which I use religiously.

In this tiny suitcase I also fit other non-negotiable items like a large bottle hairspray for my newly cut bangs that I DO NOT know how to style correctly and a some paper and pens for writing lots of good notes! I was extremely proud of myself for fitting everything I needed into one small suitcase because that meant I could avoid checking my bag.

When I arrived at the airport I started to panic because although I planned on not checking my bag, I did not really think about what that meant. I kid you not, I started to sweat. What did I even pack? All of my skin care products have to be over the 3oz limit. Would I have to leave them behind? Would I have to pick and choose what to keep and what to get rid of?

Of course everyone knows what is coming....my bag got stopped during the security process at the Columbus airport. This always happens to me. I must have had a look of guilt as I wonder if they are going to dig around in my bag and find my tweezers. They were not concerned with my tweezers, although they should have been because anyone who owns a pair of tweezerman tweezers knows that they could substitute for a weapon in a pinch....pun intended!

They were also not concerned about my skin care products…thank god because I would prefer to miss my plane over throwing those babies out. I actually know someone who politely told security at the airport that she would rather miss her plane than throw out her skincare products because they cost more than her plane ticket.

What did not make it through airport security was the aerosol hair spray. Since I don’t know how to style my bangs anyway, I will just pin them back. Crisis averted. Throw it out!

I got to the hotel late Thursday evening and I was exhausted but excited for the weekend to begin. I was not only excited to go on this trip because it meant 2 nights at an amazing hotel and fantastic views of Carlsbad (although that didn’t hurt ☺), I was also looking forward to meeting the people responsible for making some of the products that Dr. Clinton and all of us at Timeless Skin Solutions really believe in. I was not disappointed. It ended up being a great opportunity for me to further my knowledge regarding skin care, acne and anti-aging in general, but it also provided me with the opportunity to talk with the SkinMedica’s research and development team about the science and clinical studies behind their products. During the course of the weekend I spent hours getting advanced training on all things skin…It was great to learn different uses and applications for various skin rejuvenation ingredients and products which we at Timeless will pass along to our patients.

The staff of Timeless Skin Solutions is committed to give our patients the best in non-surgical, skin rejuvenation options and will continue to attend training seminars not only in Columbus but in other parts of the Country. Personally, I have to say that attending a conference on skin care, knowing that I will be able to help Timeless patients look their best, did not seem like work!

50 at 30 - to keep me 30 at 50!!!

Alas, it's time again! Every 3 months it's time to re-do your Botox or Dysport injections to keep those wrinkles at bay. In fact, just last week Dr. Clinton injected me to keep my 30-something face looking the best it can. 50 units at 30 years old - so that I'll be looking like I'm 30 when I’m 50!

Botox and Dysport are both injectable solutions used to smooth wrinkles which are caused by muscular contraction. And, yes, they both work and they are both great! Both products work by temporarily relaxing the muscles of facial expression which are responsible for drawing the skin together and creating lines or wrinkles.

We use Botox and Dysport to treat frown lines between brows, fine lines on the forehead, eyes and lips, as well as “bunny lines” on the sides of the nose, the fine lines around the mouth and chin dimpling. Among other things, these products can also be used to smooth “necklace lines” and prevent the lines that result from the down-turning of the mouth. The treated muscles will progressively relax over the course of the several days following the injection and will reach their maximum degree of relaxation within about 5-10 days. The effects of Botox or Dysport gradually start to wear off between 2-3 months after the injection and most patients find that to maintain their treatment they need to be re-injected about every 3 months. As always, these figures can vary greatly depending on the patient.

At Timeless Skin Solutions, the cost of Botox and Dysport is priced by the unit rather than by the area as some other practices do. Each patient has their own individual needs for optimal treatment and we feel that each patient’s needs are best met with the exact amount that is appropriate for each patient's unique situation, i.e. muscle strength, patient desires and severity of lines or wrinkles.

So, what are the differences between Botox and Dysport? Well, to be honest, not too much! Botox has been used effectively for cosmetic purposes in the United States for many years, whereas Dysport’s approval for cosmetic use has been more recent (although it has been used safely for many years throughout Europe and many other countries worldwide). However, while they are similar in function, they are not the same drug. They have different dosages and may effect patients differently. While both compounds have good patient satisfaction, some patients have noticed clinical differences such as duration of onset and duration of effect.

I use the Clarisonic Opal morning and night around the eye area – and when I am out of the eye serum contained in the Opal I simply use my Obagi Elastiderm Eye Gel - another favorite. After I am done with the eye area I move on to my upper lip area. Although the Opal is designed for around the eye I figure it can’t hurt to infuse a little product into the upper lip area where women my age start to form lines!

The Clarisonic Brush is perfect to remove that extra layer of sunscreen I have been applying to my face all summer along with any dead skin cells that need a bit of help sloughing off. Better yet the products I am using can penetrate my skin better making them 30% more effective.

Amanda’s Pick …

I have been dealing with small breakouts off and on for about 6 months now and it took quite a while to find something that worked well for my skin. After trying to self medicate and do things my way, I took Dr. Clinton’s advice and found haven in the SkinMedica Acne Toner and the Derma-RX Blemish Spot treatment. I have to be honest, someone with more severe and persistent acne problems would probably need a more revved up skin care regimen, but this turned out to be the perfect combination for me. The Purifying Toner contains 2% Salicylic Acid and is gentle and non-drying. I use it twice a day right after washing and it leaves my skin with a nice tingly clean feeling. The Blemish Spot treatment is a really neat little concoction made of Sulfur, Salicylic Acid and Zinc Oxide. I use it at night before bed and just sleep in it, and if I wake up with a new pimple I put it on while I’m getting ready and take it off before putting on my makeup. It really dries up my pimples fast and I love it!

I have always had acne prone skin, so thick luxurious moisturizers or sunscreens are on my list of things to avoid. It wasn't until I came across SkinMedica’s AHA/BHA Cream that I was able to introduce a moisturizer into my life! This cream boasts the power to exfoliate, revitalize and protect skin with a combination of 15% alpha-and beta-hydroxy acids as well as antioxidants. I started using it at nighttime to help with some of the texture and dryness that I was getting from other acne fighting products and now I’m hooked! Freshen Up Your Face!

Putting on foundation can be a blessing and a curse. It can temporarily cover up uneven skin tones, be a great base for eye make up, or if you're like my two male roommates, it's a great thing to take from my make up case to cover up a zit before a date (of course they never remember to return it!). For me, unfortunately, it can also cake onto the fine blond "peach fuzz" that I have on my face. Considering that lasers used for hair removal are unable to remove these fine hairs I was okay with just ignoring this minor annoyance (to be removed by a laser, hair you want removed must have pigment, so blond, red, gray and white hairs are not good candidates for laser hair removal). In my spare time I enjoy reading about our procedures I love learning about new things that I can do in terms of beauty and keeping my skin looking great. Dermaplaning was one of the very first services that I wanted to try.

Dermaplaning is a quick 15 minute appointment where a specialized tool is used to both lightly exfoliate the skin as well as remove fine hairs to leave a fresh, smooth surface behind. Not only did I love the results immediately after, but later on in the evening when it was time to do my normal nightly skincare regimen, my products had less to pass through to absorb into my face. Dermaplaning is now a normal part of my skin care regimen and I try to make sure I get a dermaplane at least once a month.

In addition to this procedure I wanted to treat my skin at a deeper level. Since I had a very busy weekend ahead of me it was suggested to me to try a very light chemical peel. Our Illuminize peel is a chemical peel we offer with a very small amount of peeling but the resulting benefits are amazing! All my life I've had fairly oily skin that seems to make my face feel heavy. Between my Obagi skincare regimen, regular dermaplaning and chemical peels my face feels fresh and bright. I previously had a much deeper chemical peel and peeled moderately on days 3 and 4 of that peel, but with the light Illuminize peel I only peeled mildly on spots where I had active acne.

Needless to say, with two exfoliating treatments my skin looked fantastic for my crazy weekend (mine and my mother's birthday). If you're looking for some beginner treatments to introduce yourself to a better looking (and fuzz free) face then this is definitely the way to get started!

My Own Dysport Challenge

If you're anything like me you're always looking for the next best thing (the iPhone 4 has basically ruined my life, my 3Gs will never look the same to me), but when it comes to choosing injectables, I want to know everything that's available to me and what it can do. Medicis, the company that makes Dysport, has a promotion right now called the "Dysport Challenge" where they offer rebates for trying out their product instead of the competing anti-aging injectable - Botox. With the option of "loving it" or "leaving it" I figured it was time to give it the old college try.

As my 25th birthday quickly approaches (tomorrow, AH!) I've been treating myself to preventative-aging procedures to take care of some of the fine lines that I had on my forehead. I was so happy with the results from those treatments I figured it was time to move on. I have a bad case of what I like to call "Jay Leno Chin". The muscle on my chin is so strong that it both makes my chin look larger than it actually is and makes the skin below crease under into an unattractive line that from a profile view can be mistaken for another chin (thanks for passing that on, Dad). Dr. Carol took me into her room and after two small injections I was on my way to subtracting a chin.

The rumor out there is that Dysport has a faster onset than Botox. In our practice we've noticed it's a Coke/Pepsi situation, it works differently for different patients, some people like it better than Botox and vice versa. At first I was confused as far as how many units I needed. Dr. Carol explained to me that I would need 2-3 times as many units of Dysport as I would with Botox, but with the lower price of Dysport will make my final total comparable to Botox. With the current Dysport challege (a $75 rebate!) it gave me the incentive to try this product and decide for myself what worked best for me. Whoever is injecting you (Dr. Carol or Allyson) can give you a better idea of how many units you would need, which product would be best for your body and what is the most cost effective for whatever your budget may be.

I've had the Dysport in my chin for about 3 weeks now and though others may not notice the difference, the way I feel about the way I look is monumental. As far as the "loving it" or "leaving it" put me down for love!

Medical Grade vs. Over-the-Counter Skin Care

Skincare is the first step to improving your skin. However, in order to get results, medical grade skincare is superior to products which can be purchased over-the-counter at the grocery, pharmacy or department store. Medical grade products are made up of proven ingredients that are purchased with a medical license and only available through physicians. These products are developed through clinical research and the amounts of active ingredients (what's actually going to make changes in your skin) are much higher than what over-the-counter products are legally able to contain. Simply put: higher amounts of active ingredients = better skin!

Think of it like buying a new Kate Spade bag at Nordstroms verses buying a Kate Spado bag on the streets of New York. You get what you pay for, so to speak, and with medical grade products you are getting A LOT more! Medical grade products contain the purest form of performance ingredients and are created from the newest technology. Hence, true cosmeceuticals are not sold over-the-counter. Over-the-counter products are often sold at lower prices, but only at the expense of cost cutting in raw materials which ultimately sacrifices the quality and efficacy of the product.

The other major difference between medical grade and over-the-counter skincare is the ability of the product to penetrate the skin. Many over-the-counter products sit on the surface of the skin, while medical grade products penetrate the epidermis to work deeper in the skin. Just like whitening your teeth at the dentist office verses useing whitening strips from Wallgreens - medical grade prodcuts can do so much more than what is available over-the-counter.

Medical grade products are worth it! They are safer, more efficacious, better quality, and proven through science.

Take Some Time Off...... Your face!

The more I look into the mirror, the more I see my grandmother. Don't get me wrong, my grandmother is one of the coolest people I've ever met, she plays the drums, knows karate and brought my mother into the world. However, being the age I am and seeing wrinkles appear around my mouth and eyes, not so cool. Unfortunately the good times that made the lines on your face, don't exactly make your face look so good over time.

If your fear is going under the knife luckily it's not a necessary fear. There are solutions beyond going under that truly work. Most of us are concerned about the lines that most call the parentheses, the lines that frame your mouth and deepen over time. The best answer for a problem such as this is facial filler. Dermal fillers such as Juvederm and Restylane will last 6-9 months and can take 10 years off of your face. Plumping up the volume doesn't only have to be around your mouth though. Do you long for Angelina's pout? Filler can also be injected into the lips to keep your smile looking much younger.

As for the crows that are dancing around the corners of your eyes, ward them off with my personal favorite, Botox. Botox usually lasts for about 3 months and luckily injections to treat crow's feet are normally the most inexpensive. Botox injections begin to take effect in less than a week and dermal fillers will show a difference right away.

Injections not really your thing? Well they should be (and again, if you haven't read my previous blogs then you'd know I'm a reformed needle-phobic) and there are a few topical problem solvers that are finally out there THAT WORK. One is SkinMedica's Essential Serum. Although the name seems more like a gimmick than a promise, if the fact that Jennifer Aniston swears by it helps then just know that! The Essential Serum is a growth factor serum that contains antioxidants and peptides, it honestly is 7 skin care products in 1. SkinMedica actually found a way to improve fine lines, wrinkles, tone and overall texture of the skin in a pump action cream dispenser.

Another "essential" product is the Clarisonic Opal. Anyone out there in the internet world have a Clarisonic Brush? Same company. How about a Sonic Care Toothbrush? Same company. And if that isn't enough for you to go out any try one I'm not exactly sure what to tell you. The Opal comes with two packs of Sea Eye Serum. Both packs last you a total of 90 days, the device has a charging base that can hold a charge long enough for you to go on your summer vacations without having to over pack your make up bag. The device itself is about the size of the palm of your hand and has a button on the side that times out 30 seconds for each of your eyes. Use it for the fine lines around your eyes, and even though this is an off label hint, some of the girls around the office have been using it for the fine lines and wrinkles around their mouths.

So if you're truly looking to take some time off, maybe vacation isn't the answer (come on, the wrinkles we get are from our significant others and our kids anyway right? You know you'll just get more if you all "get away" at once!) Relax at home and start a new skin care routine that will make you look like you've always just come back from a relaxing retreat.

Kate’s boyfriend … Laser Hair Removal

"Shave my neck". How many times has your husband or boyfriend asked, "Will you shave my neck please?" This is not an unreasonable request but I still find myself thinking, sure, I will absolutely shave your neck, when it’s not 10:00 at night and I’m busy finishing up with the dishes, reading a journal article or more likely watching Grey’s Anatomy! It seems to be a source of frustration for him and he has expressed multiple times his interest in getting rid of the hair for good! I am not unsympathetic to my significant others plight. We all know how fast hair grows, some of us shave in the morning and by the end of the day we think to ourselves, did I really forget to shave my legs again? Laser hair removal is the perfect solution for removing unwanted hair from men’s necks, backs and shoulders. It’s also perfect for us women to remove unwanted hair from our lips, chins, bikini area and legs just to name a few!

Men tend to have thicker oilier skin and significantly larger pores than women do. Basically, this means in order for men to keep their pores clear and their skin healthy and looking good men really need to cleanse their skin morning and night, although as we know many men do not take the time to do this. I have one of "them" at home who much to my utter dismay has been known to wake up in the morning and head out the door without even washing his face. His biggest hesitation is mostly time versus using products that he views as feminine. Needless to say, a regimen comparable to mine or yours is just not going to happen! Much to my surprise the one “product” I use and was unable to stop him from using is my Clarisonic brush. In his words " It’s just easy to use and I can tell a difference when I use it." The Clarisonic brush can actually be used in the shower where most men like to wash their faces anyways. It is waterproof and holds a charge for up to a week so it’s not inconvenient or time consuming in the least.

I have been using SkinMedica’s Essential Serum for over a year now and I absolutely love the results. I combine it with my Obagi skin care routine and I could not be happier with how my skin looks. Lynsey’s boyfriend … Laser Genesis

My boyfriend recently had a work related accident that left what I believed to be a small scar above his right eye. He was very self conscious about the scar and decided to come to Dr. Clinton. Even with one laser genesis treatment, he's thrilled that the scar is less noticeable and is already feeling much better about something that I barely noticed. This treatment also led him to start asking questions about the products that I was using for my face. Even though my bottles of Obagi product are emptying more quickly because my boyfriend is discretely using my product, I'm glad that between the products I have at home and the treatments he's scheduling in the office that he's feeling better about himself (and maybe I can even get him in here for the things I want him to try out!)

Sun Safety Information and Tips

Dr. Carol wants to make sure everyone understands how to be safe in the sun. Please review the following information regarding Sun Safety and Ultraviolet Index (UV) Alerts.

Sun safety tips for everyday
Please be sure to do multiple layers of these tips on days that have a UV Alert.

Remember the "S" for sun safety:

Slip on some sun-protective clothing that covers as much skin as possible.

Slop on broad spectrum, water resistant SPF30+ sunscreen. Put it on 20 minutes before you go outdoors and every two hours afterwards. Sunscreen should never be used to extend the time you spend in the sun.

Slap on a hat that protects your face, head, neck and ears.

Seek shade.

Slide on some sunglasses. Recommend sunglasses that reflect or filter out 99-100 % of UVA and UVB light, with wavelengths up to 400 nanometers (nm). Sunglasses which meet this requirement are often labeled as "UV 400.

A UV Alert is issued when the UV Index forecast is 3 or above, a level that can damage your skin and lead to skin cancer. The higher the index value, the greater the potential for damage to your skin. Extra care should always be taken between 10am and 3pm when UV levels reach their peak.

Please note the following conditions that increase the amount of UV light:
1. Time of day (peak 10am-2pm)
2. Reflective surfaces such as water or snow and sand
3. Cloud cover has a small diminishing effect on UV radiation
4. Closeness to equator
5. Altitude

Always make sunscreen part of your daily skin care routine!

For those of us who have grown up in Dublin or lived in Columbus, the Memorial Tournament has special meaning. To some of us it marks the beginning of summer break from school, breezy evenings dining with friends or the much anticipated family vacation. Spring Clean your Skin Care Routine!

Now's the time to give your skincare routine a spring cleaning. As you start cleaning out your closet, you also need to do the same with your skincare drawer. Skin care products have a "shelf" life just like other products. The active ingredients degrade over time due to exposure to sunlight or variation in temperatures. To get the full benefit of your anti-aging treatments, you want to be sure that your products are new and fresh for the best results.

To transition your fall and winter skincare routine to a spring and summer regimen consider the following options:

Switch to a lighter moisturizer to accomodate for increasing temperatures and increased oil production in the skin
To reduce the risk of acne breakouts due to increased sweating in the summer heat, add a toner or consider switching to a toner with 2% salicylic acid. For many of us, it takes a “major event” to spur us into action. For me, it was the wedding of my son, Christopher. If you're like me, you despise tax season. I normally do my own taxes because in my mind I just cannot justify paying someone to tell me what I should be paying someone else. With the way the economy is today even Jim Cramer is making mistakes. I'm pretty sure he's still kicking himself for not investing in NetFlix.

With all of the things that we can be wrong about when it comes to our money one thing that you can never go wrong with is spending it on something that makes you feel good. Televisions and computers are outdated within weeks, clothes only fit a certain way for so long, and make-up and hair products expire before you make the life change to really use them everyday. So after sifting through all of the things that might not be the best idea what are we left with? Our skin and our bodies! You'll only have your one body and of course our skin changes with us. Here are some of my favorite ways that Timeless can help you make you feel good about spending your return (or making yourself feel better after you've had to pay the government!)

Botox: If you read my last blog (and if you didn't, shame on you, I was told it was entertaining) then you know how thrilled I was with my results. I'm about to begin month three and my strong eyebrow muscles still haven't been giving me the terrible wrinkles that I was used to frowning at in the mirror. The injections are incredibly quick and if you've been furrowing your brow since January over the IRS, furrow no more and save yourself the future wrinkles!

Chemical Peels: I cannot rave enough about chemical peels (and the fact that they're 20% off through April 16th doesn't hurt either!). I had my chemical peel on a Thursday evening and by Monday morning I can't count how many compliments I received not only about how my skin looked, but some people didn't even know what was different about me! My favorite quote: "Lyns, I don't know what you did differently, but you look really great", and keep in mind that the compliment didn't come from my significant other who is required by relationship law to compliment me. Expect to peel on days 3 and 4, but with a light peel I was comfortable with being out in public house hunting and grocery shopping.

IPL (Intense Pulsed Light): I adore this treatment! I'm a fair-skinned, freckle-faced, reddish-hue hair colored mess. The IPL procedure helps to even out skin tones, hyperpigmentation (freckles, sun-damaged skin), and reduces redness. My favorite part was there was absolutely no down time. I was out and about within ten minutes of my treatment, if you're even a little concerned with how you'll look, my fair skin was a tiny bit pink, as if I had a slight sunburn. The color on my face faded quickly and my freckle count seemed to go down after a few days.

Laser Hair Removal: The sun is shining, skin is showing and honestly, that means you might need to shave everyday and how annoying is that? I personally have not done any laser hair removal, but it's something I'm ridiculously interested in because of the results I've seen on others. My mother has had laser hair removal previously and is very happy with how her treatment area turned out. Now hair "removal" is best described as a permanent hair reduction. The biggest thing to remember when you are having this treatment is not to wax or pluck the area your unwanted hair dwells. The laser fixates on the pigment of the hair. The easiest way to do a quick self check on whether or not your hair is appropriate for laser removal is if the hair itself is darker than your skin. Unfortunately hair that is blond or gray/white would not have the same results as brunette or black hair. So shave away but don't pluck! So while you're finishing up all of your tax forms (and make sure anything you send is postmarked by tax day, because that's another headache you don't want) think about all of the great things you can do for yourself. And Jim Cramer might think he knows everything about your money, unless it's about Verizon becoming a mainstream cable provider (a 2008 prediction if you're curious) or how it truly is better to invest in something solid, like yourself.

Truth Time: I Have a 5 O’clock Shadow

For me, summer time means tank tops, bathing suits, and warm starry nights – nowhere in this romantic picture do I ever envision dealing with a 5 o'clock shadow on my armpits. This is hardly what I would call attractive. Shaving is tedious, can leave behind uncomfortable ingrown hairs, and when I shave in the morning, I'm already dealing with the beginning of little dark hairs starting to grow back by night fall…..maybe even by lunch time.

Since working at Timeless, I have been lucky enough to receive treatment for this little problem o' mine, and the results have been fantastic. Because of the way it works, you need at least 5 treatments to successfully reduce your hair growth, and the benefits honestly outweigh the minor disadvantages.

So, what are the advantages?

1. No more hair!! Need I say more?

What are the disadvantages?

1. It isn't painless – People say it feels like a rubber band snapping against your skin, and I think that's a fair assessment, but in my head I compare it more to someone touching your skin with a hot lighter for a second. Not quite the same image that a cute little rubber band gives you, but that's how I see it. The good thing though, is that the pain is very short-lived, we give you numbing cream to put on before you come in, and in 10 minutes (about how long it takes to have your underarms treated) you're in and out of here and you can go on about your day and forget it even happened.

2. It cannot treat hairs without pigment - So if you have light blond hairs or white/gray hairs, you are not a candidate for laser hair removal. It is unfortunate, but because the laser is removing the hair by targeting the pigment in it, the light hairs you have will not go away.

All in all, laser hair removal is 100% worth your time and money. Anything that means saying goodbye to annoying stubborn hairs and hello to smooth and silky skin once and for all sounds good in my book.

The Staff at Timeless Shares their Chemical Peel Experiences

When it comes to chemical peels I have always thought Samantha Jones from Sex and the City and her crazy red face! For those of you who watched the show, you know what I am talking about! With the image of Samantha in my head I had serious concerns, ok maybe even fears, about getting a chemical peel. I was relieved to learn that while there are peels that can leave you looking like this, there are also milder peels that do a great job of freshening up the skin.

My chemical peel experience went as follows:
While Allyson applied the chemical peel I experienced a slight burning sensation. This was followed by itchy skin for several hours later. I took a Zantac, yes the medicine for heartburn, to stop the itching. My skin was pink the day I received the peel. The following day my skin looked pretty normal. Day 3 and 4 my skin was peeling and flaking but looked fine as long as I kept a bit of moisturizer on it. The key is to have a moisturizer that does not irritate the skin at this point. I used SkinMedica's TNS Ceramide Cream. I am now on day 5 and my skin looks glowy and radiant! I plan to make chemical peels part of my overall skin care plan!

My skin has been uncharacteristically broken out lately and I've actually been pretty down about it – I was lucky enough to never have suffered from acne in the past, so this has been an interesting lesson for me. Allyson suggested that I do a SkinMedica Illuminize Peel to start out because I'm not very "good" at the kind of peeling you have to go through with the deeper peels. The Illuminize is a really light, superficial peel that doesn't actually make you shed all that much skin. It has alpha hydroxy acids, phytic acid and salicylic acid and then Allyson topped off the peel with some retinoic acid to give it a boost. The retinoic isn't always added with this peel, but I'm glad she chose to do so because I think it has given me a better result. Today is day 3 of my peel, and I'm still a little peely, but my skin is way smoother than before and all of my zits have dried up! This probably isn't the peel for a more severe acne patient, but for someone who wants a quick pick-me-up, little downtime, and smoother, clearer, more rejuvenated skin, it's definitely the way to go.

Staff Education at Timeless Skin Solutions: Chemical Peels

Each month at Timless Skin Solutions we have a staff meeting where I take some time to provide staff education over one of our treatments, products, or dermatologic issues in general. This past month we reviewed chemical peels because everyone at the office was getting them! Below are some of the specifics (probably more detail that you want to know!) of what our staff education entailed.

Uses:
Chemical peels can be used on the face, neck and body, including the décolleté and back. Chemical peels can be used to treat a wide range of skin conditions. Some of these include: acne and mild scarring, a more youthful, refreshed and radiant appearance to skin texture and tone, restored sun-damaged, wrinkled, or blotchy skin and younger, tighter and fresher looking skin. It is important to note that chemical peels cannot treat all issue; deeper lines, wrinkles and other flaws will require additional intervention.

Mechanism of Action:
Chemical peels are used to improve the skin's appearance by applying a chemical solution to the skin, causing the top layers of skin to dehydrate, separate and peel off removing the skin's damaged outer layers. The precise formula and depth of a peel can be modified for each individual's needs. Patients will note immediate improvement after 1 peel, and dramatic results after a series of peels with little downtime.

Comparison to other treatments:
We can enhance the results of a chemical peel with laser/light-based rejuvenation techniques, or combine with another procedure, such as dermal fillers or Botox. In comparrison to microdermabrasion or Silk peels, chemical peels utilize a chemical solution which is applied to the skin to remove the surface tissue. During a microdermabrasion we use a mechanical method to achieve a similar result. The goal of both procedures is to rejuvenate and exfoliate the skin. A chemical peel, however, has the ability to go much deeper than physical microdermabrasion.

Treatment duration:
During a chemical peel, most patients experience a warm to hot sensation that may last about 5 to 10 minutes and may be followed by some stinging. The skin will become pink/red, which is followed by scaling that lasts three to five days. Sometimes Retin A or alpha hydroxy acid is used to pre-treat the skin. This thins out the skin's surface layer, allowing the chemical solution to penetrate more deeply and evenly.

Complications/Contraindications:
Patientswho are allergic to salicylates or aspirin should not receive a chemical peel. Other patients who are not candidates for a chemical peel include patient's taking Accutane, pregnant or breastfeeding patients, patients with excessive sun exposure, patients with active herpes simplex, warts, history of keloidal scarring, or patients who have recently waxed or used a depilatory such as "Nair" or "Vaniqua." There are several complications that could occur following a chemical peel. Do NOT ever pick or pull at the flaking or peeling skin, it can cause scarring or an infection. Certain skin types also have a risk of developing a temporary or permanent skin color change. Hyperpigmentation (excessive coloration) and hypopigmentation (lack of pigmentation) can be problematic of misdiagnosed skin types, failure to reveal any skin problems or ethnicity and improper post-peel care.

Limitations:
While chemical peels are a wonderful tool to refresh, rejuvinize and improve skin texture, quatity and tone, they do have their limitations. Chemical peels cannot tighten loose or sagging skin, remove deep scars, change pore size or remove broken blood vessels on the face; however, the procedure may improve the appearance of these conditions.

Vacations can be fun, exciting and let’s not forget…stressful!

Today is the day before I embark on my vacation, a western Caribbean cruise! I am trying to get my bags packed, my camera and ipod charged and not forget any of the items I cannot live without. Last week I asked the other Timeless staff members to not only give me advice for packing, but to let me know what products they cannot live without! Most of us are on a full skincare regimen, and with airport security, it is tough to take it all! Take for example one of our patients who had their full Obagi regimen in their carry on and when stopped by airport security and told to “throw it away” she said she would rather miss her flight!

So what should you take on vacation?

My last trip was to Grand Cayman. I don't leave home without my Clarisonic Brush! The charge lasts about 3 to 4 weeks and I would be lost without it. My last trip was probably to Spain? I can't remember because it’s been so terribly long since I went anywhere but I am going in August so I can let you know what I will be taking with me! I always pack a good moisturizing cream for the plane ride because my skin gets dried out …I like SkinMedica’s Dermal Repair for my face and Chapstick (my favorite-is l'occitane with shea butter). I don't travel anywhere without my Clarisonic Brush - the best part is that it holds a charge for up to 3 to 4 weeks. Spring Break Travelers Take Note!

STOP the use of your retin A, retinol and hydroquinone about 5 days before departure if your destination is to a sunny location! These creams make your skin more sensitive to the sun and let's face it, those of us that live in Ohio have not been exposed to repeated days of direct sun for months now!

The specific products that you need to be aware of include the following:

Tretinoin is a Retin A.
Differen or Tazorac, these are retinoids.
Epiquin is hydroquinone in the Skin Medica system.
Obagi #3, #5 and #6 are hydroquinone in the Obagi system. Note that there is a #6 in the Obagi system that is a pure sunscreen. If you are unsure which Obagi #6 you have please check for hydroquinone as its active ingredient.

You should resume these products when you return from vacation. If you have any questions please feel free to call the Timeless Skin Solutions staff at 614-799-5100.

While out in the sun, remember to seek shade, wear a hat and keep your sunscreen applied! Don't forget to pack your Clarisonic brush and Opal – you will miss them more than your dog!

Roots and Skin Care Have More in Common Than One Would Think...

I was getting ready to go to dinner with my valentine and all I could think about is my hair and..its roots! Then I got to thinking about the idea of maintenance…which got me thinking about a question patients often ask me!

“How do I keep my skin looking healthy and radiant!?”

The reason looking at my hair got me thinking about this questions is because someone once told me that maintaining your skin is much like coloring your hair.

Right now I am about one month over due for an appointment with my hairdresser. I know this because I have 1.5-inch roots, and somehow I did not realize it until today! Of course, I may be the only person to notice, but if I don’t “maintain” the color, the highlights grow out, they fade, and my hair looks dull. Similarly, if you do not maintain your skin, it may look ruddy and on the day of an event or romantic holiday, you may look in the mirror and wonder…how did I just notice this?!

Here are some suggestions to those wondering how to achieve healthy radiant skin...and how to maintain it!

Establish a daily regimen:

The most basic at home regimen consists of using a medical grade facial wash, toner, exfoliant and a moisturizer. Depending on your concerns we will add or take away from this regimen. Sun Screen that protects against UVA/UVB rays are important anytime you go outdoors, the UVA/UVB rays cause wrinkles and damage to the DNA of your cells causing sun damage.

Make a monthly pit stop: recharge your skin!

In the office, we recommend regular Silk Peels/microdermabrasians and chemical peels at least twice a year. Being new to the office I didn't quite know how to feel about injections and laser treatments. All I had ever truly known about treatments like Botox were falsities that SNL would spew about wide-eyed emotionless women with permanent half-smiles that were incapable of producing tears. I was happy to find out and witness first hand that this is absolutely not the case in terms of procedures here at Timeless. So I got to go through the new patient process, not only to learn how to better take care of my skin but to also address some of the things about myself I'm less than confident about. If you've read this blog or have been to our office before I'm sure you've heard or read Dr. Carol's tag "look like yourself only better" this can not be more true in my case.

First I had my consultation with Allyson (our PA-C) and not only did I feel comfortable talking about what I see as physical short-comings (which is a pretty big deal for me, outgoing on the surface, but in all honesty a fairly closed off person underneath) but she was honest about what I could and couldn't do to fix things I was concerned about. We figured out a skin care regimen to get rid of some moderate acne (yay Obagi!) and started to delve into actual procedure options. For me this was not only foreign in my world but incredibly scary. What if I don't look like me when we're done? You can't reverse Botox, you just have to wait it out. IPL? You think we should laser my face?! Why would you want to laser my face?!

After she explained how the procedures she recommended and the ones I was interested in worked it was time to move on to some of the actual treatments. I was still a little skeptical about how a laser would feel on my face and how I was going to handle injections in my forehead considering my needle phobia. The IPL was going to refresh my face take away some of my dead skin just to give me more of a fresh start. It was a warm feeling and just made me a little jumpy every time I was "zapped". After we were done with that I had just a slight redness but it was nothing compared to some of the epic sunburns I've had on my fair skin. And the redness disappeared within a half an hour.

Next, the big one, Botox. I'm a person that has a lot of movement and expression in my eyebrows. Only being 24 I had already accepted the fact that as I age I was probably going to be cursed with deep wrinkles in my forehead and between my eyebrows (or the "elevens" as I learned they were called). I laid down and when I was told how many units were being injected I was pretty shocked to know how much I needed. In reality though the number of units is actually more than the number of needles actually needed for the injections. A small relief for the needle-phobic. All I could really feel was gloved fingers on my forehead, 95% of the time I had no idea needles were going into my skin unless I could see it right above my eyes. It was an amazing learning experience that I'm incredibly thankful for.

Bottom line for all of the readers: you don't need to be afraid of needles, consultations or procedures here at Timeless. There were even some procedures that I was interested in that in my initial consultation Allyson told me were completely unnecessary. In addition to loving my new job it's nice to know that there are still businesses that are more interested in the best interest of their employees and patients and not the extra stuff they can slap on.

Pump up the volume!

As the number of days shortened before my 47th birthday I noticed what many people notice as they age – the loss of volume in my face!

It is a simple fact that as we age we lose facial volume. This loss of volume tends to be the most noticeable around our mouths, eyes and lips. Although Dr. Carol has injected filler into my nasolabial folds, which run from the side of the nose to the corner of the mouth, as well as under my eyes, I had not yet tried filler in my lips. To be honest the thought of having filler injected into my lips was scary! I was fearful that I would look like either Lisa Rinna or Daffy Duck – neither of which was very appealing to me.

Even with the thoughts of Lisa Rinna rolling around in my head, every time I looked in the mirror I was very aware of my disappearing lips and I really wanted to do something about it.

The plan … I decided to trust Dr. Carol, as I have so many times before, and have my lips injected. Dr Carol injected 2 ml of filler into my lips and around my mouth. Was I scared? Yes, but I knew I was in good hands with Dr. Carol as she is a top injector of filler in the area.

The result … I could not be happier! My lips look fantastic! It’s just as the Timeless tagline reads: “Look like yourself only better”. Perfectly stated. My lips don’t look crazy, actually they look perfectly natural. I followed both the pre and post procedure instructions and had minimal bruising and swelling. Acne Management

This Friday, February 19th, from 12:00-1:00pm, I will be holding an informational seminar on acne. Below is an idea of some of the things I will be talking about. Please stop by if you have any questions!

Many people, young and old, male and female suffer from acne. Acne breakouts range from low inflammatory bumps and papules, to more severe inflammation with deep, red, protuberant cysts.

Commonly used over-the-counter products are frequently ineffective, and tend to dry out the skin, causing rebound oil production rather than cure the underlying source of the breakout . Medically directed skincare provides a proven advantage over these over-the –counter options. These products are purchased with a medical license and have a higher percentage of active ingredient to treat to area of concern.

For all skin types, Clarisonic has an at home ultrasonic cleansing brush which helps with deep pore cleansing. For dry or sensitive skin, cleanse with an mild cleanser, followed by a beta-hydroxy (salicylic acid) preparation in the form of a toner or lotion, and moisturize with a vitamin B hydration lotion. For very oily skin, the amount of acid in your wash can be increased. If your breakouts are isolated to just the forehead or shoulders, it may be a reaction to a hair product. It may also be pityrosporum, which responds to anti-fungal treatment.

If you have acne, it’s best to avoid wearing makeup if you can. But if you must, choose a makeup with a mineral base, such as zinc and magnesium, which have a calming effect on the skin. When you buy makeup, ask if it’s preservative-free, so that it soothes your skin and doesn’t clog pores.

If acne is not resolving with the above measures, silk peels and/or chemical peels are the next step. Both help prepare the skin by removing its surface and uncovering the follicles so that topical preparations can be absorbed. These two treatments can be done together for a quicker response, but the resulting dryness and peeling will be more noticeable.

Remember, when treating acne, it may get worse before it gets better. Two days after a chemical peel, the skin will flake and acne can flare for about two weeks. A skin cycle is 28 days, so you won’t see the full effect of any treatment for about a month.

If you have greater acne inflammation, or significant cyclical breakouts, you can try light-based therapies. These include a series of blue light treatments to absorb a by-product of the bacteria in the skin or Levulan with photodynamic therapy. Following Levulan, the skin will redden, peel and flake for about 3-7 days. Acne may flare after the first treatment, but a series of three treatments usually results in permanent changes to the skin. This treatment is also effective for cancerous and pre-cancerous lesions.

Prescription topical or oral antibiotics can be effective with used in conjunction with topical treatments. Commonly used antibiotics include clindamycin, erythromycin, and minocycline. Azaleic acid also has antibacterial activity and can improve hyperpigmentation that sometimes occurs with acne. Topical retinoids, such as Retin-A™ and Differen™ are also often prescribed. Oral contraceptives and Spironolactone can regulate hormone-related breakouts. Accutane™ is reserved only for the most severe cases of acne that haven’t responded to other therapies. This is a drug with potentially serious side effects, and all the facts should be considered before initiation of this regimen.

What would Dr. Clinton do?

Well, if you are wondering "what has Dr. Clinton been up to?", I thought I'd share my venture into a surgical experience in January of 2010. I have been bothered by some extra fat under my chin since I finished chemotherapy about five years ago. I had to take steroids at that time and the only thing that really got "pumped up" was the fat under my chin! So, I spoke with plastic surgeonDr. Brian Dorner in November. We decided proceed with liposuction of the chin and neck and he also recommended an upper and lower lid procedure (blepharoplasty) and a forehead lift. As the last three procedures were beyond what I had initially sought the in the consult, I considered his professional advice and agreed with all of the procedures recommended - with the assurance that I'd look better but not "surprised" or "hollow" or looking like a clown. The procedures took 4 hours. I was in excellent hands during the procedure. As I requested to be awake during the procedures, I received valium and local anesthetic. I would not recommend "being awake" for the faint of heart! The liposuction of the neck was by far the easiest part of the journey. The process to recover was much more intense than the nonsurgical alternatives, so if you are considering doing any surgical procedure, please be prepared for some "downtime" that goes beyond just "social downtime." The swelling and bruising have resolved at this time and I am quite pleased with my results and the care I received from Dr. Dorner and his staff. In preparation for the procedure, I had a Titan treatment of the neck and have followed it up at this point with a chemical peel. I plan on adding an IPL, Pearl, Filler and Botox procedure shortly. I used my Skin Medica TNS Essential Serum during the healing phase as well as my Clarisonic Brush and Opal. TSS Investigates - DIY Botox

If anyone knows that value of a dollar, it’s me! I’m a coupon-clipping, rewards card-holding, price comparing and negotiating, black Friday shopping maniac! But one sacrifice I would never make – putting my health in danger just to save a few bucks. In my eyes, it’s just not worth it.

I came across an article the other day from the Lincoln Journal Star reporting on a burglary at a plastic surgeon’s office. It was your typical, everyday robbery – “…stolen items included a flatscreen TV, a laptop computer, two digital cameras and cash…” and “…six boxes of botulinum toxin type A,” AKA Botox! Wait a second, did I read that correctly? They stole $3,150 worth of Botox? What would two of Lincoln’s finest do with Botox vials?

“Well you know people are illegally selling Botox on the internet now?” Kristy later informed me. That sparked my interest – and I immediately started my search to find Botox for purchase online – without a prescription & without an MD at the end of my name.

I discovered a fantastic article by Lorraine Fisher from the Dailymail.com, with, what I think, is quite a show-stopping title “Poison-by-post.com : The terrifying new trend that could paralyze, blind or even kill you.” I mean if that doesn’t scare you, what would?

Lorraine did all the leg work for me – she researched sites online where you can purchase Botox and actually bought some. It seemed so easy - she simply searched “buy botox”, a website appeared, she selected three vials and headed to the online check-out. The Botox soon arrived, without any instructions on how to transform this gel-like substance into the liquid Botox we all see when at our physician’s office. (In case you aren’t aware, Botox is actually shipped in solid form and the physician combines that with sodium chloride before injecting a patient).

This might seem all fine and dandy up to this point – but let’s just bring to light some of the more “shady” things Lorraine ran into during her purchasing experience. The first, and most obvious red light is the fact that you can only pay with Western Union or Moneygram transfers. And even further, the money was to be sent to some lady with a fake name in Chisinau, Moldova. If Western Union asked you what the money was for, you were told to lie and tell them the money is “…for your relatives or for a friend in need.” Lastly, another little known fact about Botox is that it must be kept cold – in fact it’s packed and shipped in dry ice when sent to our office from Allergan. But was Lorraine’s illegitimate botox packed in ice? Nope, it was sent in a plain old envelope, so after all that, if by some miracle the product she received was really Botox, by the time it got to her, it wasn’t good anymore.

As I read through more articles, what seemed to interest me the most was not the physicians’ opinions on DIY Botox but instead the comments left by the public about the subject. Comments ranged from one end to the other; from citizens concerned for those who would take such a drastic step just to save a few bucks; to comments defending these online illegal drug stores; to those who think we should just all mind our own business and let people do as they please. I even saw a post from a past employee of one of these websites claiming her innocence of the whole situation.

I'm not trying to make up anyone's mind about this - after all, from what I've read, this seems to be quite a touchy subject - but I just don't see how risking your life is worth saving some money. Like any prescription you receive, there are possible side effects of Botox, side effects that are hightened when you aren’t fully trained to be dispensing the drug.

Allergan (makers of Botox) suggest seeing "dermatologists, plastic surgeons, ophthalmologists, otolaryngologists (ear, nose, and throat doctors), or other physicians specializing in cosmetic procedures [because they] are generally more experienced." On the Botox website doctors are listed for your convenience based on zip code so you can be sure you are seeing a trained, experienced physician.

So why take the risk?

No More Make-Up!

What does every girl want? ...Not to have to wake up every morning and feel like you can't leave the house without slathering on a hefty coat of make-up to hide the imperfections of age and sun-damage. Well, I am here to tell you that, thanks to Dr. Clinton and the Pearl laser, I have now officially achieved that goal!

I am now 3 weeks post my Pearl laser procedure with Dr. Clinton and I am LOVING the results! Not only are my fine lines smoothed out, my texture improved and my pore size reduced, but my hyperpigmentation has also vanished!

I can't tell you how good it feels as a new Mom to have the priceless luxury of not having to take the time to apply make-up every morning. To be able to spend 5 extra minutes playing with my new baby instead of standing in front of the mirror getting ready for work. To be able to snuggle cheeks with my little girl before leaving the house and not have to worry about what toxins from my make-up are getting on her fresh baby skin. What a gift!

NEW PRODUCT ALERT - The Clarisonic Opal! - How Do Our Patients Feel About it?

Clarisonic, the makers of the Clarisonic Brush [a staff favorite!] just came out with a brand new product using the same gentle micro-massaging movements as the Clarisonic Brush & the Sonic Toothbrush, but geared toward your under-eyes. Although the product was just released on Tuesday, our office was lucky enough to receive a sneak preview of the product about 30 days ago. We were given some Opals for us and some of our patients to try out before the product was even on the market!

We asked our “test” patients to supply us with some basic feedback of the product, based on 7 simple questions;

Was the product easy to use?

Was it difficult to add to your normal skincare routine?

Did you see immediate results?

Did you apply other product(s) either before or after use?

Did you like the serum that came with the product?

Did you use any other product in the opal, instead of the serum?

Do you have any major complaints?

At a quick glance of the responses, the feedback was great! The product was easy to use and effortless to add into a normal skin care regimen. It seemed that the big amazement for everyone was the immediate results you could see after using the Opal. One patient exclaimed, “Actually, much to my surprise…I could see an immediate difference in the reduction of small lines around my eyes!” Based on feedback, the best time to use the Opal seems to be after cleansing as you apply your facial moisturizer. Instead of patting in the eye cream you currently use, simply switch over to the Opal and in the same amount of time, you can expect immediate “plumping” of the skin around your eyes.

Another praise we heard from our patients is the futuristic appearance of the Opal itself. Not only of the sleek outer shell, but also in the trendy design of the charger – even being compared to the Wii remote charger. One patient loved “the magnetic click you hear when you set the Opal on its charger base.” Some did experience a little confusion when first setting up the charger, because it is so different than most product chargers in the industry – and for that very reason we have an Opal laid out in the office so you can play around with it before you take yours home!

A specially formulated Anti-Aging Serum is included with your purchase. The Opal has not been clinically tested using any other product besides this serum – but we know how attached some are to their eye creams (myself included with the TNS Illuminating Eye!) - so we were interested to see if some of our “test” patients used their favorite eye product instead of/or in addition to the serum. Every patient liked the serum included, noting that “It has a very nice consistency and smell to it. Very cosmetically elegant,” and “…it's smooth and disappears quickly.” No one seemed to use one of their products in the Opal, in place of the serum, but instead just used their favorite eye cream before use, and then continued with the regimen after use (i.e. Essential Serum, sunscreen, Vitamin C).

I know what you all are wondering - were there any complaints? Not one patient had a MAJOR complaint about the product, just a few small concerns or unanswered questions. First of all, one patient showed interest in learning more about the serum itself; what ingredients are in the serum and what went into the development of this serum to work hand-in-hand with the Opal. We went straight to Clarisonic for that answer, and here it is:

“Our Anti-Aging Sea Serum was scientifically developed for optimum results with the Sonic Infuser. Key points about our Serum are… specific viscosity (thickness and consistency) to work best with the cupped applicator tip [and] specific rheology (the physical way that it flows) to work with the dynamics of the Sonic Infuser.”

Ingredients include, “…Key marine and botanical ingredients to hydrate (Trehalose); to firm and tighten (Glycosaminoglycans and Kigelia Africana Extract); and to soothe and protect (Canadian Willowherb). As well, our serum has a natural preservative derived from naturally occurring substances and is Paraben free.”

Other patients expressed a need for more instructions on use, “…how much pressure to add or how to rotate…because it's so different from the Clarisonic Brush and it's close to the eye.” That is a great question, and based on our experience, both with the Clarisonic Brush and so far with the Opal; pressure is not really needed. What’s so great about the Clarisonic Brush is that the sonic vibration shakes and lifts dirt and bacteria out of the skin without much pressure at all. Same with the Opal, the gentle vibrations will plump and softly infuse the serum into your under eye area – doing all the work for you, no need to add additional pressure!

Another question that frequently arose was the expectation of how long the serum will last and how many applications you should expect with each serum. The answer there is - 180 applications, 90 per eye at twice a day – so you can expect the serum to last you 45 days. If you choose to use the product once a day, then obviously you will see a longer life from the serum – it’s up to you and your face!

Now for some fun! What other advice or insider information did our patients have for future Opal users?

“…it is great for when your sinuses are full after a nights sleep and you want to reduce the puffiness under eyes!”

“The Opal is really great for travel… It's small, looks good and comes with a great little bag. “

Latisse: What’s all the fuss about?

Believe me, I get it – as the average person watching the commercials of beautiful Brooke Shields flapping around her enviable eyelashes, you may say to yourself….“let’s be real…does this actually work, or is it just hype?” I am admittedly a person who doesn’t believe in anything they see on commercials, and I probably wouldn’t have thought twice about using Latisse after seeing the advertisements…especially with all the talk about change in eye color and hair growing in strange places. (Who would want their eye color to change?) However, because I am lucky enough to work here, and I had the opportunity to try Latisse, I fell in love. Not only that, I now know everything about it that one could never know from simply seeing the commercials. Therefore, I have set out to educate anyone willing to listen about what it really does, and what you should realistically expect when using this remarkable product.

One of the best things about Latisse is that it’s so customizable. You can use it as little or as often as you would like depending on how long you want your lashes to be, you can use it more often specifically on the outside corners of your lid to create a more doe-eyed sort of look, and we actually have heard both male and female patients comment that they have been using it on their eyebrows as well and are loving it. (note: Latisse was not tested for nor was it FDA approved for use on the eyebrows)

I also love it because virtually anyone is a good candidate. I’ve always had pretty okay lashes, but since using Latisse I have amazing lashes and I love the way they look. There are also plenty of women who have either never had long eyelashes, or who throughout the years have lost a lot of their previous length and volume. I have seen women come in with teeny tiny lashes barely visible to the naked eye, and after about 8 weeks I saw them with a much fuller lash and a lot more confidence as well. This is also a great product for women and men post-chemotherapy. Many people, Dr. Clinton included, never fully recover all of their lashes and brows after chemotherapy, and this is a really great way to help some of that hair grow back.

Does it change your eye color?

The “mother” product of Latisse is actually a pharmaceutical drug, Lumigen, which is used for Glaucoma patients. This product is of a much higher concentration, and it is dropped directly into the eye. Patients using Lumigen can experience some change in eye color (i.e. people with brown eyes may notice specks of yellow or gold appear in their iris), but this was never seen during the testing for Latisse.

Will I grow hair in weird places?

Not if you use the product correctly. Latisse comes with small applicators, so it’s hard to miss your target area, and you would have to continually put the solution in an unwanted area to create hair growth there.

Any other side effects?

There are patients who have experienced some irritation on their eyelids since using Latisse, i.e. itching and redness, and those patients have either chosen to return the product to us for a full refund or just power through it because they love the benefits of Latisse so much. It is also possible that the area where you apply Latisse could darken -- you apply it to your eyelid in the same place that you would apply eyeliner, so it was never something that scared me particularly, but it is important to know that a slight change in color is a possibility.

If I have done nothing else in this post, I hope to have at least made a few weary consumers feel more comfortable with the idea of Latisse and answered some of the most frequent questions we get here at the office. Now, if only Latisse could give me Brooke Shields’ awesome hair and incredible height…maybe next year.

“I don’t want to look crazy.”

Recently, during a consultation I was speaking with a patient new to our practice. (I will call her Sarah.) She had never done any type of cosmetic procedure in the past, so in our consultation we were talking over some of her concerns and possible treatments. Sarah shared with me, that although she had been thinking about doing Botox for a long time, that she had hesitated until now due to a slight fear of being pulled or pinched too tight. “I do not want to leave my house and have everyone thinking I look like Joan Rivers!” she explained to me.

I thoroughly enjoyed her articulation of the concern! After speaking with several current patients who admitted that before coming into our office, they had felt the same way, I started to address the concern in all of my consultations. It turns out, that many new patients, like Sarah, have the same hesitation when considering even a non-surgical procedure, like Botox.

We believe in using the laser technology and non- surgical procedures that we have available to improve and maintain the appearance of your skin. As I told Sarah, Dr. Clinton and Allyson are fantastic at taking a patient’s desired look into consideration when doing an injection or laser procedure. To make a long story short, Sarah went ahead with her treatment and sent me a note that she loved the Botox and was glad she tried something out of her comfort zone.

For those of you who may feel the same way, here are some tips on how to improve the way you feel about your skin with minimally invasive treatments such as Botox, dermal filler or laser procedures.

3 ways to get non surgical procedures and not look crazy!!

#1 Choose wisely: a physician that is experienced in doing your desired treatment and a practice that values your opinion, answers your questions, and addresses any concerns you may have. *Dr. Clinton is in the top 6% of injectors in the nation for Botox cosmetic.

#2 Be clear: when speaking with your doctor about what you are looking for, be clear about your wants/needs. Make sure that you and the medical professional are on the same page prior to the treatment. * If we are injecting your lips it is going to be for a natural looking enhancement, so if you want them as big as Lisa Rinna’s…you will have to speak up.

#3 Communicate: after treatment with your care provider be sure to communicate with them about how you feel. This way they can chart on your satisfaction and be able to maintain or improve the quality of your care during future treatments * We always offer a free follow up visit 2-3 weeks after your procedure to see how you are doing, and tweak if needed.

Mommy make-over!!

This past Sunday, Dr. Clinton treated me to a "Mommy make-over!"

During my 9 months of pregnancy, some of my hyperpigmentation returned from all the hormones circling in my body. Not only that, but thanks to my lack of sleep I saw a lot of my fine lines reemerging and was just feeling not all that "Timeless." But, now, thanks to Dr. Clinton, that is all resolved, and Mommy is back in action!

On Sunday afternoon, while many of you were cozying up by the fire, I was having the Pearl laser and a series of injections to revitalize my tired Mommy-face. The Pearl laser is used to improve fine lines, texture, sun damage or hyperpigmentation and promote collage production. Facial fillers are used to restore areas where there has been a loss of collagen...and for me that meant I got 3 syringes - 1 syringe to plump up my nasolabial folds, and 2 syringes in my lips (the better to kiss that sweet little baby of mine!).

Botox

Frequently, I am asked the youngest age that I will treat a patient. I currently have a patient who is quite young (under age 10) who has asymmetry of her smile. 2 units of Botox, every three months, even out her smile and make it easy for her to be just another kid in grade school. So, there is not an age that is "too young", but rather one where it becomes "just right." Botox is a great treatment for men and women alike. Call the office and we'll help you figure it out. There has been a lot of buzz in our office surrounding the hilarious and introspective read, I Feel Bad About My Neck: And Other Thoughts on Being a Woman by Nora Ephron. We love the honest way she looks at life and aging, and she reminds us all that every once in a while we just have to laugh at ourselves and try to enjoy the ride. One of our favorite passages from the collection of essays relays a conversation Nora has with herself that seems all too familiar, “You can put make up on your face, concealer under your eyes and dye on your hair. You can inject Botox and filler into your lines and creases; however your neck remains a dead giveaway. You have to cut open a redwood tree to see how old it is, but you wouldn't have to if it had a neck." Now, that’s undeniably humorous - but it brings us to a very important question, “What can I do about my neck?” There are a few options that we offer at our office for just that concern. We consider the best option to be the Titan 3D laser treatment. Titan works by using infrared light to travel through your skin and down to the deeper layers. In doing this, the light actually tightens the collagen architecture in the dermis. One of the many upsides of this treatment, in comparison with others, is that although some redness will probably occur immediately after treatment, there is minimal pain involved and absolutely no downtime. With the Titan treatment you should expect to see some subtle results within a few weeks, but the full results will be seen in about six months. Initially, two treatments of Titan are performed about two weeks apart, and some patients may want to do a single maintenance treatment at about the two year mark.

While the Titan is ideal for patients who are concerned with lax or sagging skin, some patients may want to have more immediate results. Many people aren’t aware of this, but Botox is actually used in the neck to relax the muscles and soften the horizontal ‘necklace lines’ and vertical ‘platysmal bands’. This is a great alternative for someone whose concern isn’t so much skin laxity, and who is looking for rapid results. Many patients choose Botox for these concerns as preparation before a big event. However, the results from the treatment will only last for a period of about three months, and would then need to be repeated to maintain.

So, ladies and gentleman, while we do not encourage you to have bad feelings about your neck, we definitely want you to know that if you do; you have options.

Allergan , Inc Given Warning by FDA: Latisse

Patients have been enjoying the effects of using Latisse since its FDA approval in December of 2008. Since that time there has been a wave of promotional material released as Allergan’s product, Latisse has gained popularity. Allergan has some wonderful marketing materials, and the results that we have seen are phenomenal. However, we should not let that overshadow the fact that Latisse is most certainly a prescription medication and that as with all Rx’s there are risks or side effects associated. The warning given to by the FDA to Allergan had to do with the fact that their promotional materials did not given prominence to the side effects, not that new side effects or other more serious problems have been encountered.

Who would think that you could treat both cancer and wrinkles at the same time? A new study out from the University of Michigan has shown this is possible!

A chemotherapy drug used to treat colon, pancreas and head and neck cancers has also been shown to reverse the signs of aging. In a small study that included 21 patients, researchers found that the drug, called fluorouracil, reduced the appearance of sun-damaged areas after being applied for just two weeks, the Daily Telegraph reported. The research from the University of Michigan shows that the drug works by causing damage to skin cells, forcing the body to rebuild them, in much the same way as laser treatment.

The study involved volunteers between the ages of 56 and 85 who applied cream made from the drug to their skin twice a day for two weeks. After the two weeks were up, dermatologists reviewed before and after pictures of the patients and found the cream had reduced both fine and course wrinkles, as well as age spots.

Fluorouracil is already used as a prescription skin treatment for actinic keratosis, which is a precancerous skin lesion that is caused by frequent and intense exposure to the sun. The findings are published in the journal Archives of Dermatology.

Other medications are available to help reverse the signs of aging, while at the same time treating actinic keratosis. A common laser procedure when combined with Levulan, a topical medication, can achieve similar results. Downtime for flourouracil will extend through the course of treatment and until healing is complete about a week after completing the course. The Levulan treatment, known as PDT, has about a 3 day period of downtime. Both treatments can be repeated over time. You can use them on the face or other areas of the body. So you pick: PDT or flourouracil and you are on your way to healthier, more youthful skin!

Out with the gaunt and tight, in with the plump and juicy. There’s a new face in town—and it’s a baby’s.

A woman I have known for many years did something to her face not all that long ago, and for a few weeks afterward, I was not able to put my finger on it. Did she get her eyes done? Restylane injections? Botox? Then I thought, Oh dear God, she got a face-lift. No one whom I consider a friend and a contemporary had yet gone that far. But there was no denying she had done something major, and frankly I was worried. Had she ruined her pretty face? As the curtain of hair slowly parted a little each week, I could see that her lips were bigger. Nowhere near overcooked-hot-dog-turning-inside-out bigger like Meg Ryan’s, and not even duck-bill bigger like Courteney Cox’s—but big enough to make me feel uncomfortable looking at her mouth when she talked. Don’t look at her lips!

Then one day, about a month later, I ran into her at a party and she looked stunning. The puffiness had settled, the fire under the skin had gone out. Even her lips looked like they belonged on her face. They were shaped just like her old lips, but … juicier. Her whole face looked as if it had been pushed out and plumped up—not unlike a slightly tired but still very stylish down-filled sofa that looks almost new if you keep those cushions fluffed. I cannot say that she looked exactly like her old self—but so close! A fantastic approximation! An uncanny resemblance! She looks like a very impressive artist’s rendering of her.

But there was also a faint likeness to someone else. She looked a little like … Madonna? Strange, I know, since Madonna and my friend have little in common, at least physically. But when I saw the Big Ciccone on the cover of Vanity Fair a couple of months later, I couldn’t help but notice the similarities: the Mount Rushmore cheekbones, the angular jawline, the smoothed forehead, the plumped skin, the heartlike shape of the face. Their faces didn’t seem pulled tight in that typical face-lift way; they seemed pushed out. Looking at Madonna, I kept thinking of the British expression for reconditioning a saddle: having it “restuffed.” Perhaps that’s where she got the idea to have some work done. After the hunt, Madge dismounted her trusty steed and thought, My saddle needs restuffing. And, by George, so does my face!

Women have been availing themselves of new faces since the dawn of plastic surgery, but suddenly it seemed that there was a better new face to be had. There is a New New Face, very different from the old one, and both my friend and Madonna now have it. Once I starting thinking of it in these terms—the face as the new handbag, say—I started seeing New New Faces everywhere: Demi Moore, Michelle Pfeiffer, Liz Hurley, Naomi Campbell, Stephanie Seymour. They all have it! Even the Olsen twins seem to have a starter version of the New New Face, with their big crazy doll eyes and plush lips. Just to be clear, I don’t presume to know exactly what any of these women have done to their faces, if anything at all. It’s possible (though in some cases before-and-after pictures would seem to suggest otherwise) that this face is occurring entirely naturally—after all, these are women who are famous for being beautiful. The point is that there is a noticeable aesthetic shift happening in the face, and that it’s dovetailing with quantum leaps in plastic surgery and dermatology.

Through some unholy marriage of extreme fitness and calorie restriction (and maybe a little lipo), women have figured out how to tame their aging bodies for longer than ever. You see them everywhere in New York City: forty- and fiftysomethings who look better than a 25-year-old in a fitted little dress or a tight pair of jeans. But this level of fitness has created a new problem to which the New New Face is the solution—gauntness. Past a certain age, to paraphrase Catherine Deneuve, it’s either your fanny or your face. In other words, if your body is fierce (from yoga, Pilates, and the treadmill), your face will have no fat on it either and it will be … unfierce. It was only a matter of time before a certain segment of the female population would figure out how to have it both ways, even if it means working out two hours a day and then paying someone to volumize their faces, as they say in the dermatology business. As a friend of mine recently pointed out, there is now a whole new class of women walking around with wiry little bodies and “big ol’ baby faces.” And they look, well, if not exactly young, then attractive in a different way. A yoga body plus the New New Face may not be a fountain of youth, but it’s a fountain of indeterminate age.

AFTER the band was chosen and the napkins color-coordinated to match her shoes, Kacey Knauer, a bride-to-be, had another critical matter to address: her skin, and the skin of the nine women in her bridal party.

So Ms. Knauer, the 35-year-old owner of TempTrends, a staffing agency in Manhattan, invited her nearest and dearest — including her mother and future mother-in-law — for a night out at the TriBeCa MedSpa, replete with mimosas and cupcakes. An aesthetician assessed each woman’s face and devised a treatment plan — a quick chemical peel, say, or an injection of a wrinkle-filler. Or maybe, for a bridesmaid with age spots, a series of Fraxel laser treatments over months, allowing for recovery time.

For Ms. Knauer, who will be married in December, cosmetic interventions for herself and her entourage are as vital as the centerpieces or food. “If I were 25 or 26 and getting married, a bracelet, necklace or matching earrings would be fine,” she said.

But at 35? “Giving them a bracelet isn’t as special as spending an evening together. Plus, as you get older, everyone is more conscientious about their skin and appearance,” she said. “Giving them something for themselves — as opposed to something that they’ll never wear again — is more meaningful.”

And let’s not forget the pictures of college roommates-turned-bridesmaids quickly posted to Facebook. It is no longer sufficient to hire a hairstylist and makeup artist to be on hand the day of. Instead, bridal parties are indulging in dermal fillers and tooth-whitening months before the Big Day.

Some brides pick up the tab for their attendants, replacing the pillbox inscribed with the wedding date with a well-earned squirt between the eyes. In other cases, bridesmaids — who may quietly seethe about unflattering dresses — are surprisingly willing to pay for cosmetic enhancements. “Most women, when they come in here, they want it,” said Camille Meyer, the owner of TriBeCa MedSpa. “They know they’re aging.”

For Karen Hohenstein, who held her party at the Tiffani Kim Institute Medical Wellness Spa in Chicago, convincing her friends was as smooth as a Botoxed forehead. “It wasn’t me saying, ‘Hey, we all could use a little something,’ ” she said. “It was, ‘I want to do this,’ and a couple of people said, ‘I do, too.’ ”

But for every accommodating pal, there’s another who feels going under the knife is beyond the duty of bridesmaid. Becky Lee, 39, a Manhattan photographer, declined when a friend asked her — and five other attendants — to have their breasts enhanced. “We’re all Asian and didn’t have a whole lot of cleavage, and she found a doctor in L.A. who was willing to do four for the price of two,” said Ms. Lee, who wore a push-up bra instead.

Not for nothing are some maids known as slaves of honor, but this kind of cajoling is a recent development on the wedding front.

Marie Scalogna-Watkinson, the founder of Spa Chicks on-the-Go, a mobile spa, said she receives five to seven calls a month from brides seeking Botox or Restylane for their bridesmaids. Five years ago, collective makeovers were unheard of, she said.

Dr. Fardad Forouzanpour, a cosmetic surgeon in Beverly Hills, Calif., said his business has increased more than 40 percent since he began offering what he calls Bridal Beauty Buffets in 2006.

In the last two years, bridal party tuneups have increased roughly 25 percent, estimated Susie Ellis, the president of SpaFinder.com, a site that lists 4,500 spas worldwide.

Just as timing matters when it comes to securing a hall, it’s best that brides-to-be don’t delay scheduling appointments, aestheticians and doctors say. “You wouldn’t get a cut and color the week before,” said Dr. Jessica Wu, a dermatologist in Beverly Hills who advises coming in three to six months before the big day. “We do a trial run of Botox about four months ahead of time. Then, two weeks before the wedding, we do that last treatment.”

Ms. Meyer of TriBeCa MedSpa suggests that a bride contact her the minute the question is popped. “Brides really appreciate the fact that we put everything in a regimented schedule for them,” she said. Since February 2007, she has staged more than 30 bridesmaid parties and has 18 planned so far this year. “If you have to do eight treatments, six weeks apart, that could take up to a year,” she said.

Fraxel laser could also set you back $1,200 a session, which even without the economic downturn, amounts to quite a bit. These days, Robyn Bomar, an event planner in Destin, Fla., overhears brides doing cost-benefit analyses. “They will never choose Botox over a great dress, but they will say ‘Maybe I’ll have a buffet over a sit-down at the rehearsal dinner,’ ” she said. Or: “I’ll spend the money on Botox rather than lunch.’ ”

Obagi & Tretinoin (Retin-A) in the Summertime

For those of you who have been using Obagi and/or Tretinoin (Retin-A), you may have noticed that you are much more sensitive to the sun now that the summer months are upon us.

Tretinoin is made from vitamin A. As many of you already know, side effects of Retin-A include dryness, excessive redness, and peeling of the skin. But, what you may not know, is that Retin-A can also increase your sensitivity to sunlight and make you more likely to get sunburned. So, be sure to protect your skin from exposure to the sun by wearing broadband sunscreen and protective clothing.

Even though I work with patients everyday to correct the damage their skin has undergone from the sun, I too like to enjoy life and get some sunshine. So, to protect my skin while I am enjoying some rays I use Anthelios SX SPF 15 as my everyday sunscreen to ensure that I am protecting my skin from both UVB (burning rays) and UVA (aging) rays. I also add Skin Medica’s Environmental Defense Sunscreen SPF 30+ and my UPF (Ultraviolet Protection Factor) 50 Sunscreen Hat when I know I am going to be enjoying the outdoors for an extended period of time.

If you find that you spend most of summer outdoors and are becoming too sun-sensitive and/or peeling too much, you may want to consider using Tretinoin every other night and Obagi #4 Exfoderm every other day instead of every day. This really made a difference for me as I started to notice my own increased sun-sensitivity once the spring sunshine hit.

I even have some patients who I recommend completely stop using their Tretinoin during the summer due to their active outdoor lifestyle with their families and children. For these patients, it is still important to continue the exfoliation process, so we replace their Retin-A with some other type of non sun-sensitizing exfoliation agent, such as Skin Medica’s 15% Alpha and Beta Hydroxy Acid Moisturizer.

The bottom line is if you use Tretinoin or any other retinoid derivative (Retin-A or Retinol), never forget your sunscreen!!!

Improve Sun Damage Layer by Layer with IPL

Take a look at your arm and count how many freckles you see. Chances are you’re wearing a lot more freckles than you did as a child. Sun damage resulting in freckles, age spots, sun spots and moles can pop up anywhere on the body - especially areas that are frequently exposed to sun light. Sometimes that damage can lead to serious skin issues including skin cancer. But treatments like Intense Pulse Light Therapy (IPL) can remove the damage leaving healthy, refreshed skin. Dr. Carol Clinton of Timeless Skin Solutions in Dublin says IPL is very effective in the treatment of sun-related aging.

“The advantage of IPL over other types of pigment and vascular removal methods is that it is fast, the recovery time is short, and the risk of side effects are low,” says Dr. Clinton. “The procedure simultaneously addresses a variety of common concerns including broken capillaries, rosacea, redness, age spots, and even enlarged pores.”

IPL therapy uses an intense light that passes through the top layer of skin and is absorbed by brown, black or blue pigment particles within the skin. The energy emitted by the laser light causes the pigment cells to break into miniscule particles which are removed by the body’s immune system.

On Friday April 25 Dr. Carol Clinton and her staff hosted the first Memorial Tournament BeFORE and After Celebration. Just like celebrities prepare months in advance for the Oscars, more than 200 women from around Central Ohio attended the celebration to prepare for the event of the summer, the Memorial Tournament. Dr. Clinton, the leading beauty doctor in Dublin, hosted the event to introduce new and current patients to the latest advancements in skin care and sun protection. Representatives from Allergan (Botox), Kinerase, Obagi and others provided procedure and product demonstrations while the Timeless Skin Solutions staff offered free patient consultations.

"This is the first Timeless Skin Solutions BeFORE and After Event. It's to help people in Dublin get educated about skin care and to get their skin ready for the Memorial Tournament. We want to make sure their skin is protected when they're out there in the sun," says Dr. Clinton.

Those in attendance learned that if you're going to be in the sun there are a couple different ingredients you need to look for in sun protection. Number one is micronized zinc and titanium. These ingredients are actually sunblocks. Then you want to look for the strongest UVA protection, which is protection from the AV rays of the sun. One month into Obagi and doing great!

I am now one month into my Obagi Nu-Derm System for oily/normal skin with 0.1% Tretinoin and my skin has never looked better!

Now, I have to admit that at first these products put my skin through the ringer! The first 2 weeks my skin peeled, itched, and was a nice shade of pink. In fact, Dr. Clinton looked at me one day and said…”Allyson, have you been playing with the Chemical peels again?!”

By week 3, I began to see some lightening of my sun damage, as well as brighter quality to my skin tone. My skin was still tight, flaky and pink, but not quite so sensitive. I did a light SilkPeel with hyaluronic acid infusion to gently exfoliate the superficial dead cells, while hydrating the deeper layers of my skin. I also began using Kinerase ProTherapy Cream as the final step in my nighttime regimen in order to add a little bit of moisture back into my skin.

I have now been using the products for one month and my skin seems to be adjusting to my new Obagi routine. I still have some days where I am a little more dry and pink, and other days were my skin seems to be just fine. I have been using my Clarisonic Skin Brush with my cleanser in both the morning and evening which has really helped to gently exfoliate away any of the dead cells that are ready to come off the surface of my skin. The best part is that I am really seeing some great changes in my skin color and texture. I have definitely noticed a more even and clear color, as well as a silkiness that has never been there before. I have to say, I’m loving it!!!

For those of you who are just getting started with Obagi, here are a few recommendations I’d like to share from my experience over the past month.

1. Avoid using #2 (Exfoderm Forte) around the eye area. This product has a combination of alpha-hydroxy acids which has quite a bite when applied around the eye area where the skin is thin and delicate.

2. Use caution when applying Tretinoin (Retin-A) on or near the corners of your mouth and near the nasolabial folds. This tends to be an area of increased sensitivity, so don’t be surprised if you notice peeling and redness in this area first. It’s normal!

3. Don’t forget to extend your products to your neck! You want the improved color, texture and tone of your skin to flow evenly from your face to your neck.

4. Don’t skimp on your sunblock! You want to make sure you are using #6 Sunfader as the last step in your morning application every day, even when it is cloudy. I even go the extra step and use my La-Rosche Posay Anthelios SPF 15 moisturizer as the last step of my morning routine. Not only does this provide better protection against UVA rays which can penetrate through windows and clouds, but it also gives my skin a little extra moisture which it can definitely use while I am adjusting to the Retin-A.

That’s it for now. I’ll report back and let you know how I’m doing as things progress. I can’t wait to see what my skin looks like a few weeks from now!

Do You Have Porexia?

Allure Magazine recently published an article highlighting a growing quest for smaller pore size. The term "Porexia" was given by a professor of dermatology to describe our preoccupation with a desire for "impossibly small pores". In the pursuit of perfect skin, many consider large pores undesirable. Smaller pore size does reflect light better and gives a more youthful appearance. Pore size is largely determined by genetics, skin type and age. As we age, we start to lose collagen and elastin that help to support the pores causing them to sag around the edges and look larger (imagine what a deflated inner tube would look like).

We must remember that pores are a biologically necessary component to our skin and to get rid of them is impossible. However, there are some things that can be done to improve the size and appearance of them making them far less noticeable.

Over the counter products will help them to look clearer but once you stop the product, the pores will return to their original condition. Exfoliating cleansers with salicylic acid, lactic or glycolic acid all stimulate cell turnover and help to clean pores. Moisturizers should be oil-free with the addition of Vitamin C or retinol to increase cell renewal and collagen production. Sunscreen is a must and needs to offer both UVA and UVB coverage. Masks with algae firm the skin and masks with clay will absorb oil.

Unfortunatley, many people do not notice dramatic improvement with over the counter products. Timeless Skin Solutions provides a variety of products and procedures, only available through a physician's office, that offers more aggressive options in our quest for smaller pore size.

Our Obagi product line contains a system of products that work together to improve overall skin texture and tone. The prescription strength Retin A in this product will give more dramatic results than over the counter retinoid products to improve cell turnover. The system also includes prescription strength Hydroquinone which will improve any dark spots or hyperpigmentation in the skin that might have been caused by previous blemishes or sun damage. The Toner portion of the system will help with oil control and the exfoliator and sunscreen will help with cell turnover and help to prevent any future sun damage respectively. Our TNS SkinMedica system will give similar results as the Obagi line but with a less aggressive form of Retin A, retinol. This would be more appropriate for people with sensitive skin. The short UVA rays penetrate deepest into the skin and are the rays of ultraviolet light that can pass through glass.

Our physician strength chemical peels combined with regular medical grade microdermabrasion (the Silk Peel in our office) will promote exfoliation to improve overall texture and rejuvenation of the skin. Intense Pulse Light combined with Laser Genesis will target uneven tone and decrease sebaceous gland activity to reduce oil production. This in turn will reduce the tendency of the pores to be clogged with sebum (oil) which leads to cleaner and smaller appearing pores. The IPL component of this treatment also moderately stimulates collagen which will reduce the laxity of the skin and will help to support the edges of the pores. The Titan 3-D procedure is combination of the IPL and Genesis along with the addition of the Titan laser. The Titan laser targets the fibroblasts in our skin which are the cells responsible for the production of collagen and helps to stimulate their activity. As we age, the fibroblasts slow down in their production of collagen which cause skin laxity. The skin laxity is usually most evident in the jowl line and neck but it also affects the edges of the pores as mentioned above.

All of the above products and procedures offer significantly more benefit than just reduction in pore size which we will be happy to review during the consultation process.

DESPITE WARNINGS, TANNING BED USAGE DOUBLES*
Experts say kids are tanning – and getting cancer – at a younger age

Some tan to add a little color to their skin. Some say it makes them look and feel healthier. Whatever the reason, Americans are using tanning salons in record numbers. By the end of the day, another two million people will tan under indoor lamps.* That number has doubled in the last decade.* But more and more young people are learning that “healthy” glow can come with serious risks.

The FDA first started regulating indoor tanning products nearly 30 years ago.** Despite constant concerns about its safety, tanning is more popular than ever, especially with teenagers.
But before they get a chance to buy into it, Shari Croy has a warning for her grandchildren. For decades, Shari worked to keep her skin golden brown. When the sun couldn’t do it, she did.
“I started out tanning every day. I had my own tanning bed for 7 years. I called it my “Florida room” downstairs,” says Croy.

Now, at 46, Shari is battling skin cancer. Doctors say after decades of intense exposure, cases like hers are not uncommon. What is surprising – a rash of new cases involving much younger patients.

“Before, we may have seen people more in their 40s. Now we’re seeing people in their teens – people who are 15 coming in with melanoma,” says Kari Kendra, MD and Ph.D. at Ohio State University’s James Cancer Hospital and Solove Research Institute.

Dr. Kendra says tanning beds are partly to blame. According to the Skin Cancer Foundation, a recent study shows using them before age 35 can increase your risk of melanoma by 75%.*** The problem is, many young people just don’t know how powerful the tanning bulbs really are.
“Even when compared to places like in Australia where the sun intensity is so much higher, the radiation from those tanning beds is still higher than the sun,” says Kendra.

It’s a message Shari got too late, but she shares it with anyone who cares to listen.

“This is what I have to say to those kids. I would show them my scars and my bald head and say it’s definitely not worth it. Definitely not worth a pretty tan when it changes your life so dramatically,” says Croy.

Croy is now working with lawmakers in Ohio to try to make it illegal to tan before you are 18. Nearly half of all states have no regulations when it comes to minors using tanning equipment.****

This weekend I took a big step. I went make-up free! I have relied on concealers and foundation and powder for more than a decade and on Saturday I decided to go without makeup. Every time I looked in the mirror I expected to see a blotchy complexion. Instead, I noticed a pretty even skin tone. This stuff must be working!

Now more than a month into treatment I think my skin has maintained the improvement I saw a couple weeks ago, but I'm not seeing additional results. The discoloration from pregnancy and acne has faded but not to the extent I thought it would. Perhaps I'm disillusioned by the images I see in magazines. I'll never be a poster girl for clear skin, but perhaps as I continue with the the SkinMedica line I'll continue to see baby steps of improvement. Like I mentioned in previous posts, these products don't give me clearer skin. In fact, I think I've had a slight increase in acne and irritation. But, the tone is better. I noticed in another post that Jennifer Aniston is a fan of TNS. Since my use of TNS is in conjunction with other products, it's hard for me to say if it's working or not. But if it will help me look as young as she does 10 years from now, I'm going to stick with the program.

I'm now a couple weeks into my treatment and I can say I haven't missed an application. Overall I think the look of my skin is improving. The color is starting to look more balanced and my skin is smooth. The only complaint I have, which probably isn't attributed to the products, is that my acne has gotten worse. I typically have a few bumps around the sides of my nose and chin, but now I'm seeing regular acne in those areas. Frustrating! I have had a head cold for the last week and a half, so we'll see once the cold subsides if my skin improves too.

I continue to see good results with the products. My skin tone is slowly improving and I haven't had any side effects. However, I do think my acne breakouts are consistent with what I was experiencing before starting this routine - no better/no worse. The next time I see Dr. Clinton I'm going to ask her if there is something I could/should be doing for the breakouts. I don't want to use too much product.

I'm a week into my new skin care routine and so far so good. I've had a mild acne breakout this week, but that's to be expected as the products I'm using aren't designed to treat acne, they're for discoloration. I don't think the products are aggravating my current acne condition. I've had it all my life. I do know it's not making it any worse. That's a pleasant non-side effect! The color and tone of my skin is improving. I can't wait to see the improvement continue. I'm eager to go makeup free once the weather gets warmer.

Today is day five of my treatment. I've followed Dr. Clinton's instructions perfectly and I can say that I'm starting to see a difference. I'm not ready to head out the door without makeup on, but I can see subtle improvement. The intense redness is starting to fade and my skin's texture is smooth. As with my experience with past products, I expected to see my acne flair up. Don't you always have to look worse before you look better? With this system, you don't. Perhaps it's because I'm treating the side effects of unbalanced skin instead of treating acne. My oil production isn't as intense and my makeup goes on smoothly.

Another note about these products is that they don't have an intense scent. The 2x daily process is pleasant and I'm eager to see the improvement continue.

I've been a guinea pig for various acne products since I was 12 years old. I've tried every over-the-counter remedy, microdermabrasion, vitamins, harsh prescription topicals, etc. On occasion I would see good results, but they didn't last. I always led myself to believe that as I aged, the acne problem would go away on its own. Now at 30, I know that's partially true. My acne breakouts aren't as bad as they were in my teens and early 20's. But today I have a whole new set of skin issue that I didn't anticipate. The years of acne have left an uneven skin tone. Combine that with many summers working as a lifeguard and skin discoloration from pregnancy and you get a pretty big mess. Anyone who has skin issues knows the toll it can take on you physically and emotionally. Before my wedding five years ago I was concerned that a breakout would ruin my big day. I've probably spent thousands of hours applying concealers, foundations, highlighters, etc. to cover up what I didn't want people to see. Today, I hope the SkinMedica TNS Regeneration System can help me take charge of my skin.

I never go to bed with make up on and I cleanse my face twice a day. I also apply Anthelios SX sunscreen to my face every morning. I would give myself an A+ for maintenance. So I think I'll be a good subject for this test. Last night I began my regimen by cleansing with SkinMedica Facial Cleanser. Unlike other cleansers, it didn't dry out my face and it also didn't leave my face feeling oily. It just felt clean. Then I applied one pump of the Retinol Complex from the SkinMedica TNS Regeneration System. This product didn't rub on smoothly like I expected it to. I don't think there's anything wrong with the texture...it's just not what I expected. My third step was to apply SkinMedica's EpiQuin Micro skin lightening moisturizing cream. This product felt great. Similar to acne products like Oxy, the lotion can rub off on fabrics and take away the color - so I was sure to sleep on an old pillow case.

When I woke up this morning my skin wasn't oily or dry. It felt balanced. My morning regimen included washing with the facial cleanser and using one pump each of the TNS Rgeneration System TNS Recovery Complex and Vitamin C Complex. I then followed up with the Anthelios SX sunscreen. So far so good. The products didn't make it difficult for me to apply make up.

It's now 11 a.m. (4 hours after this morning's applications) and my skin isn't oily. Of course the color hasn't improved just yet (that would be a miracle), but so far I'm satisfied.

When I woke up this morning, I could still feel the tautness in my skin from the application last night. It was almost midday before I noticed that I still had the same feeling. I took a shower after applying the cleanser. This time around I followed it up with the pore therapy application. It states on the instruction card to use any cotton ball or square. It made me wonder why I could use any normal cotton with the pore therapy but I had to use the specialty cotton squares with the serum gel. Maybe the specialty cotton squares where indeed part of the overall treatment as I concluded from my first entry. This time the serum gel application feels more like regular rubbing alcohol. If you have ever rubbed or massaged your face with rubbing alcohol, like they used to do in the old days, you will have an understanding as to what I'm referring. After the application, I could feel it working its way deep into my skin and after awhile that minty eucalyptic feeling is back on. Glancing at the mirror, I noticed that my nose and especially my forehead isn't shining like before, hmmm. Dr. Clinton also recommended that whenever I go out that I apply some sunscreen on my face. I decided to stay in today so I didn't use the Anthelios SX Daily Moisturizing Cream with Sunscreen that she recommended. Like before this is not an endorsement of Anthelios SX or of any La Roche-Posay products.

My skin gets oily easily causing me to break out during the winter months. When I break out, I get little baby pimples mostly on my forehead close to my hairline and around my mouth...especially when I don't shave. On occasion I'll get one or two in my nose. Typically I have one or two pimples and not a full breakout. I've encountered common problems when using popular acne medications available at drug stores. They normally would dry up the initial breakout but I'd often find new pimples right next to the location of the old pimple...or the pimple would just return. Ssometimes they tend to form the results of a linear equation! Aside from my face, I have regular acne on my scalp, which thanks to Dr. Clinton, I just discovered can be treated with Selsun Blue. I was referred to Dr. Clinton by my wife who recommended using Obagi. I've been using the product for a little while. I've missed some treatments and I feel like my face just got oilier. With recent weather conditions, my face feels like it did the first time I visited Dr. Clinton.

Here is my treatment routine: Around 10 p.m. I apply some daily care foaming cleanser to my face. It feels like a body wash. After rinsing it off, my skin feels taut and after an hour it feels like I just washed my face. Next I use the serum gel with the enclosed cotton pads. The pads don't feel like ones you can get anywhere else. They feel like they have a special coating. After the application, my skin starts to tingle deep down. If you ever spilled acid in a chemistry lab and it got on your skin... well, the feeling is about the same but without the burn. No wonder they have a disclaimer about temporary tingling or stinging during the first week of usage.

Recent Article About Botox

An article was brought to my attention yesterday about the FDA looking into adverse reactions to Botox and Botox Cosmetic that have been blamed for hospitalizations and even deaths -- mostly in children treated for cerebral palsy-associated limb spasticity, an off-label usage.

The FDA does a wonderful job of providing a feedback mechanism for unusual events, as evidenced by the recent press with Botox. Please know these adverse events were in low body weight (usually very young children) with severe major muscle spasticity. Large doses, in ranges far above what is used cosmetically, were used in these children. Botox is still considered a safe and effective treatment for facial muscle relaxation. Botox is delivered for these indications in small doses for small muscles. Botox is a medical procedure and should be treated as such. While the risk profile remains low, it is not advisable to receive Botox from a professional who considers the administration of Botox “a party”.

I’m in San Antonio and reviewed all the new products at the American Academy of Dermatology. Fraxel introduced its new CO2 technology that was mentioned on NBC’s Today Show last Friday. I’ve had several patient’s ask me about this. Here is some information.

This is a fractioned laser source that treats only part of the skin. It has less down time than CO2, which was one of the first full face rejuvenating lasers on the market. This is a very aggressive laser and it goes quite deep. The idea is to give a CO2 result (which in many cases leaves the skin slightly pink in appearance) without the downtime of a full CO2 treatment. It is a painful procedure and although CO2 is the gold standard, most physicians have moved away from this technology due to downtime, discomfort, and complication profile. This is truly a “burn care” post procedure and quite often there is significant bleeding and oozing. It is not a procedure for the faint of heart. And, if you are going to do a fractionated procedure, why not do the gold standard full CO2 procedure? I don’t see this as a huge improvement in the aesthetic laser market; just modifying existing technology a bit.

Cosmeceuticals seem to have the biggest splash at the AAD. The trend is to recognize that these high grade products should either begin just before procedures but most certainly should be used after procedures to give the greatest benefit to the health of one’s skin.

All Procedures Have Risks

Last week it was reported that BBC newsreader Kate Silverton is taking legal action against a leading surgeon after undergoing Fraxel laser surgery that left her face so badly marked she was unable to work for two weeks.

She's quoted as saying, "It's been awful. I went in to get some minor scarring on my cheeks treated...I was told it would be a routine procedure and I'd be back to work in days. The treatment, however, caused a massive skin reaction."

All procedures have risks. It is unfortunate that Kate had to endure this. Some people experience Fraxel as a no downtime procedure. However, the best use for the procedure, to repair acne scars, typically does require some downtime due to the depth and energy used in the procedure. When using higher energy, the risk for post inflammatory hypo and hyperpigmentation rise significantly. Swelling and redness can also be quite severe. Time and topical creams can improve discoloration, but results may be permanent.

Pearl, Perlane, and The Palette

Here we are at the end of another exciting week. On Sunday, my husband woke up and asked to have Perlane and the Pearl. While it would be fun if these were names of new positions, they are the names of new procedures! Perlane was used to fill up areas that had lost volume and Pearl was used to sandblast years of sun damage and aging. By today, he looks fantastic. Those deep lines that were frown furrows are mostly gone and many of his fine lines and sun damage has been erased. Chris turned 50 this year and looks great. So keep in mind, these treatments are not for females only! Chris has not yet requested the Palette, but I am eagerly awaiting mine. The Palette is Mukha's solution to all of your make up needs.

I've talked about Anthelios since last summer and the FDA is reacting to the fact that labelling needs to address the UVA damage, which are the rays that cause aging and skin cancer. The Season of Giving - Home Tour

As my practice continues to grow, I am asked to support many different charities and functions. I am happy to do this and it seems the functions continue to become more diverse and yet reflect many of the passions that I have. This year I have sponsored several youth sporting events and teams, including Eli Pinney Elementary, St. Charles Baseball and Soccer, the Dublin Charity Cup, Dublin Coffman Volleyball and the Nike Cup Soccer Tournament. As I am a frequent listener to NPR on WCBE, I sponsored a donor challenge on two afternoons, first to my fellow medical professionals and another one for my patients and friends. I have suported the American Cancer Society and the National Ovarian Cancer Coalition. I support Scholarship Opportunities for Success which aids education for women of low income. Over the holidays, we are opening our home for the Dublin Women's Club Christmas tour of homes. Our family home will be on tour on Sunday, December 3, from 12N to 5PM. Our home gives an exciting twist to the tour, as it is a Frank Lloyd Wright style home and sits on two acres. Tickets for the event will be sold at the spa and throughout Dublin. The Dublin Women's Club supports education for local students and the Ronald McDonald House. This is the 25th year of the tour. Finally, the Central Ohio Diabetes Association is holding a Deck the Hall Tour of homes in Upper Arlington. This event is dear to my heart, as I lost a beloved brother-in-law to diabetes several years ago. I am sponsoring a scholarship through this organization. This event is held on November 30.
